Position Summary The Case Manager supports court-appointed professional guardians and conservators in providing person-centered, ethical, and legally compliant services to adults under guardianship. This role coordinates care, monitors client well-being, maintains accurate records, communicates with families and service providers, and helps ensure that each client's needs, preferences, rights, assets, and court-ordered care plans are appropriately addressed. The Case Manager does not make decisions outside the authority granted by the court or delegated by the appointed guardian and must maintain strict confidentiality, professional boundaries, and compliance with all applicable laws, regulations, court orders, and company policies. Essential Duties and Responsibilities Assist professional guardians with ongoing case management for assigned clients under guardianship and/or conservatorship. Conduct regular in-person, virtual, or telephone client contacts, including home, hospital, skilled-nursing, assisted-living, and community visits, as required by court orders, care plans, company policy, and client needs. Monitor clients' health, safety, living conditions, care quality, service needs, and general well-being. Identify changes in a client's physical health, mental health, functional abilities, housing, support system, or risk level and promptly report concerns to the assigned guardian or supervisor. Coordinate services with medical providers, behavioral-health professionals, social workers, residential facilities, home-care agencies, benefits agencies, attorneys, financial institutions, and community resources. Assist with arranging and monitoring medical appointments, transportation, housing placement, personal-care services, benefits, and other approved supports. Maintain timely, complete, and objective case notes, visit documentation, correspondence, incident reports, care plans, and other records in the company's case-management system. Gather documentation needed for court reports, annual accountings, care-plan updates, benefit applications, and other legal or administrative filings. Communicate professionally and compassionately with clients, families, attorneys, advocates, care providers, and court personnel while respecting confidentiality and established communication protocols. Support client self-determination by identifying and documenting the client's known wishes, values, preferences, strengths, cultural needs, and least-restrictive alternatives. Respond appropriately to emergencies, suspected abuse, neglect, exploitation, unsafe living conditions, or other critical incidents; follow mandatory-reporting requirements and internal escalation procedures. Assist with reviewing bills, benefits, and service-related expenses for accuracy and appropriateness, when assigned, without exceeding delegated financial authority. Maintain awareness of clients' rights and report any potential rights violations, conflicts of interest, undue influence, or exploitation concerns. Participate in interdisciplinary meetings, care conferences, staff meetings, training, and case reviews. Maintain a professional, trauma-informed, culturally responsive, and client-centered approach in all interactions. Perform other related duties as assigned. Required Qualifications and Experience Bachelor's degree in social work, human services, psychology, gerontology, nursing, criminal justice, public administration, or a related field; equivalent relevant experience may be considered. Two to four years of relevant experience in case management, social services, aging services, disability services, behavioral health, healthcare coordination, long-term care, guardianship, or a related field. Experience working with vulnerable adults, including older adults and/or individuals with intellectual or developmental disabilities, serious mental illness, dementia, brain injury, substance-use disorders, or complex medical needs. Demonstrated ability to manage multiple cases, prioritize competing deadlines, and maintain thorough documentation. Knowledge of community resources, public benefits, healthcare systems, long-term-care services, and social-service referral processes. Strong written, verbal, interpersonal, and conflict-resolution skills. Ability to communicate effectively with clients who may have cognitive, communication, behavioral-health, or mobility-related challenges. Proficiency with Microsoft Office or Google Workspace and electronic case-management or health-record systems. Valid driver's license, reliable transportation, and ability to travel locally for client visits, meetings, and emergencies, if required. Preferred Qualifications Bachelor's or master's degree in social work, counseling, gerontology, healthcare administration, or a related field. Certified Guardian, National Certified Guardian, Certified Case Manager, social-work licensure, nursing licensure, or similar relevant credential. Direct experience with professional guardianship, conservatorship, probate court matters, fiduciary services, or court-appointed services. Experience preparing documentation for court reports, care plans, incident reports, or benefits applications. Knowledge of Medicare, Medicaid, Social Security, Supplemental Security Income, veterans' benefits, long-term-care systems, and housing resources.
