Resilire Neuro is seeking a compassionate and organized Case Manager to coordinate care and rehabilitation services for individuals with brain injuries and other complex neurological conditions. This role serves as the primary point of contact for clients, families, physicians, and interdisciplinary team members, ensuring services are coordinated, person-centered, and aligned with each client's goals. What You'll Do As a Case Manager, you will: Coordinate care plans and rehabilitation services for an assigned caseload. Serve as the primary liaison between clients, families, providers, and care teams. Facilitate care conferences, interdisciplinary meetings, and discharge planning. Monitor client progress, identify barriers, and coordinate solutions and resources. Maintain accurate documentation and ensure continuity of care. Advocate for client needs and support informed decision-making. Coordinate transitions into, between, and out of services What We're Looking For Qualified candidates will have: Bachelor's degree in Social Work, Rehabilitation Counseling, Psychology, Nursing, Human Services, Healthcare Administration, or related field. LMSW Preferred. Minimum 1 year of experience in case management, care coordination, healthcare, rehabilitation, behavioral health, or human services. CPR certification or ability to obtain.
