Full Time Salary Qualifications:

Minimum of a bachelors' degree in a human service-related field. A minimum of two years' experience working with adolescents. Previous experience working in community-based services with a working knowledge of housing options. State certification as a social worker preferred but not required. Experience in youth casework desirable. Must possess a valid driver's license, have access to an automobile with adequate personal and passenger liability insurance coverage. A working knowledge of the juvenile justice system and basic understanding of psychological, sociological, and social work concepts is desirable. Should be able to relate well to youth, parents, agency, and supervisory personnel, as well as co-workers. Must be available for a flexible work schedule beyond days and afternoons, which may include late evenings, weekends, and/or holidays.

Responsibilities:

Provide case management services, in Shiawassee and Lapeer Counties, to youth ages 16 to 24 experiencing literal homelessness, vulnerably housed, and/or at risk of a housing crisis. Provide case management and supportive services to runaway youth ages 10-17, to assist them in reintegration into their family home or housing in a safe alternative, and to reduce the number of home truancy status offenses and delinquencies among youth. Services must be provided through a Positive Youth Development approach with an emphasis on Trauma-Informed Care. Position Functions

• Conduct initial intake assessments to determine eligibility for adjacent counties' youth entry into either the Reach services or the Traverse Place prevention program determined by age of the youth and the participant's request.
• Conduct assessments and required paperwork to determine the present status and needs for each youth in mental health needs, family involvement and needs, substance use disorder, behavioral and relationship concerns, career development, educational pursuits, life skills trainings, and housing stability.
• Provide ongoing case management for the length of time the youth and family are eligible to receive services and aftercare for each youth and when appropriate, their families.
• Maintain a case file for each youth in tandem with the Reach and Traverse Place programs inclusive of the development of a case plan for each youth and family.
• Submits all necessary monthly, quarterly, and annual reports, and assists in data collection for program evaluation.
• Provides necessary community agency referrals and follow up for project participants and their families.
• Assists each youth with the steps required to accomplish the objectives within their individualized case plan goals.
• Coordinate opportunities for youth to explore career opportunities, educational programs, social service linkages, and housing options through creating contacts and organizing connections within the community.
• Complete all necessary paperwork in compliance with federal, state, and local regulations and grant/contract requirements.
• Work in conjunction with the Street Outreach Team in Shiawassee and Lapeer Counties to disseminate information to county business, agencies, social service programs, schools, law enforcement, etc.
• Participate in activities and attend regular meetings of the respective county homeless coalition / local planning body. Business Functions
• Work in partnership with both the REACH and Traverse Place teams to provide effective and efficient services.
• Support residential staff activities and provide back-up shift work when necessary.
• Provide input for changes in program policy or procedures.
• Attend scheduled training and orientation sessions and seek opportunities for professional growth.
• Receive incoming calls for assistance from youth, families, or others in the community and direct appropriate staff or community resources.
