The Case Manager for the A New Hope (ANH) program provides trauma-informed case management and housing stabilization services to survivors of domestic violence experiencing homelessness. The Case Manager assists participants in securing and maintaining safe, stable housing through individualized service planning, housing navigation, and coordination of supportive services. The Case Manager works with participants to rapidly connect them to permanent housing and community resources while providing intensive case management and advocacy. Services are delivered in collaboration with community partners and are designed to promote safety, housing stability, and long-term self-sufficiency.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Case Management and Client Engagement Provide trauma-informed, survivor-centered case management to individuals and families enrolled in the ANH Transitional Housing (TH) and Rapid Re-Housing (RRH) programs. Conduct intake, screening, eligibility determination, and final interviews for referred participants. Complete comprehensive assessments with participants accepted into the program and develop individualized Housing Service Plans. Maintain regular contact with participants and provide intensive case management services, particularly during early stages of enrollment. Implement the Critical Time Intervention (CTI) model to support stabilization and transition to community-based supports. Conduct reassessments of participant eligibility and needs in accordance with grant requirements. Monitor participants' progress toward housing stabilization goals and update Housing Service Plans as needed. Provide crisis intervention, safety planning, and advocacy when necessary. Ensure participants maintain compliance with lease requirements and program expectations. Conduct regular home and employment visits to support housing stability. Follow up with participants monthly to ensure progress toward established goals. Assess housing stability and program outcomes, including follow-up after stabilization. Housing Navigation and Stabilization Assist participants in locating safe and affordable housing that meets HUD habitability standards and fair market rent guidelines. Assist with housing search, housing applications, and landlord negotiations. Educate participants on tenant rights, responsibilities, and lease agreements. Coordinate move-in assistance and housing stabilization strategies. Provide landlord mediation and support to prevent eviction and maintain housing stability. Assist participants in coordinating access to furniture and other housing necessities. Supportive Services Coordination Coordinate supportive services including life skills training, budgeting, employment assistance, transportation, childcare, healthcare, and other wraparound supports. Connect participants to mainstream benefits such as SNAP, SSI, Medicaid, employment services, and other available resources. Develop and maintain partnerships with community agencies to support participants' needs. Coordinate referrals for mental health services, medical care, psychiatric medication, financial assistance, and legal advocacy. Ensure participants are connected to services that promote long-term housing stability beyond program participation. Documentation and Compliance Maintain accurate, timely, and complete documentation of participant services and activities within the
HMIS/AWARDS
system. Enter intake data, case notes, contacts, and financial assistance information in accordance with grant requirements and organizational policies. Maintain participant files in compliance with confidentiality standards and data security requirements. Ensure participant recertifications are completed according to grant guidelines. Coordinate with program leadership and fiscal staff regarding financial assistance requests for participants. Program Coordination and Team Collaboration Work collaboratively with case management staff to monitor service delivery and participant progress. Participate in Coordinated Entry System (CES) processes and community outreach efforts. Contribute to the continuous quality improvement of MUTEH housing programs. Provide weekly updates on caseload and participant progress. Maintain successful linkages with community partners and service providers. Participate in daily debrief sessions, supervisory meetings, staff meetings, and training opportunities. Attend applicable trainings and professional development activities.
QUALIFICATIONS AND KEY COMPETENCIES
Bachelor's Degree in Social Work or related field (preferred), or Associate's degree with minimum of two (2) years case management experience. Experience working with the homeless or impoverished populations is helpful. As the case management work spans a number of counties in MUTEH's coverage area, this position requires regular travel in personal and/or agency vehicles. Case managers must have a valid driver's license, their own personal vehicle and proof of personal vehicle insurance. Ability to work with diverse populations and be able to meet clients in their homes. Excellent verbal and written communication skills. Strong organizational skills, attention to detail, and ability to meet deadlines consistently. Ability to work effectively both independently and as part of a team. Demonstrate maturity and professionalism even in stressful situations. Commitment to an empowering, client-centered approach to case management. Compassion for people who are struggling with a variety of challenges, including homelessness, mental health, and/or substance abuse. Act with integrity; exercise appropriate discretion and maintain confidentiality of information. Ability to manage competing priorities effectively.

Frequently uses equipment such as telephones, computers and printers.
