Help older adults remain independent in their own homes through expert care coordination. Northland PACE is seeking an experienced Care & Services Coordinator to serve as a vital member of our interdisciplinary care team. This position is responsible for coordinating, monitoring, and managing the non-clinical services that allow frail older adults to safely remain in their homes while receiving comprehensive PACE services. This is an ideal opportunity for someone with a background in care coordination, case management, social services, healthcare navigation, home and community-based services, or senior services. What You'll Do Assess participants' home care needs during enrollment and ongoing reassessments. Coordinate home care services such as housekeeping, laundry, shopping, meal support, and other community-based services. Arrange Durable Medical Equipment (DME) and other home safety resources. Collaborate with participants, families, and the Interdisciplinary Team (IDT) to develop and implement individualized care plans. Ensure home-based services are implemented accurately and documented appropriately. Participate in weekly interdisciplinary team meetings. Assist participants with Medicaid renewals, SNAP, housing applications, and other benefit-related paperwork. Attend medical appointments when participant advocacy or escort services are needed. Complete PASSR screenings and maintain accurate electronic medical records. Monitor services to ensure participants remain safely and successfully living in the community. We're Looking For The successful candidate has experience coordinating services for older adults or medically complex individuals and enjoys organizing multiple moving pieces while building strong relationships with participants, families, providers, and community resources. Minimum Qualifications Minimum 2 years of professional experience in healthcare, case management, care coordination, social services, human services, or a related field. Experience working with older adults, individuals with disabilities, or medically complex populations preferred. Strong organizational, communication, and problem-solving skills. Ability to work independently while collaborating with an interdisciplinary healthcare team. Proficiency with electronic medical records and documentation. Valid driver's license, acceptable driving record, and proof of automobile insurance.
