TriSource is in need of a Case Manager to provide direct services to homeless women in a large shelter setting. You would help clients identify needs, access community resources, and develop individualized plans toward stability and self-sufficiency. This is a Temp-to-Perm position, offering an opportunity for permanent employment based on performance and organizational needs. Monday-Friday 8:00am-5pm with flexibility depending on client needs Essential Duties & Responsibilities Conduct comprehensive client assessments to determine immediate and long-term needs Develop individualized service plans outlining goals, timelines, and resources Coordinate services such as housing, financial assistance, medical care, mental health services, employment resources, and other supportive programs Maintain accurate and timely case documentation in accordance with agency and funder requirements Provide emotional support and crisis intervention as needed Advocate for clients to ensure access to appropriate services and community resources Make referrals to local agencies, housing programs, employment services, and other supports Monitor client progress and conduct regular follow-ups to adjust service plans as needed Complete required reports in a timely and thorough manner Maintain case records according to agency guidelines Required Knowledge, Skills & Abilities Ability to speak, read, and write in English Demonstrated cultural sensitivity and ability to work with diverse populations At least 1 years experience in providing case management to individuals Bachelors degree preferred Must successfully pass FDLE Level II background screening Valid Florida driver's license and reliable transportation (ability to drive agency van or bus preferred)
