Job Description

Job Title:
Full-Time Carolyn's House Case Manager with Emphasis on Youth YWCA of the Niagara Frontier, Inc.
Mission:
The YWCA of the Niagara Frontier is dedicated to eliminating racism, empowering women and promoting peace, justice, freedom and dignity for all.
Job Objective:
This position is responsible for case management for homeless women and children that are residents of Carolyn's House, a 19 unit supportive housing program in Niagara Falls.
YWCA Responsibilities:
As an employee of the YWCA of the Niagara Frontier, the employee is responsible for, promoting and Mission, programs and services of the organization. Attend and participate in staff meetings and in-service trainings (occasional travel to different locations for meetings).
Accountability:
Carolyn's House Director Qualifications:
  • Minimum Associates Degree in human services or related field and a minimum of three years' experience working with clients presenting with homeless, domestic violence or chemical dependency issues.
  • Able to work with staff and clients with diverse backgrounds and age groups. Responsibilities include but are not limited to:
  • Conduct screenings of potential residents, to include mental health and chemical dependency assessment when appropriate.
  • Develop individual service plan for youth and adults and establish goals with residents.
  • Provide direct case management for residents.
  • Link residents with appropriate referrals.
  • Facilitate weekly resident group meetings.
  • Prepare and maintain case notes
  • Maintain HMIS system for residents that are non-domestic violence victims.
Pay:
$43,500
