Job Description

Transition, Housing, & Independent Living Specialist The Specialist will spend their time assisting seniors & people with disabilities who have Medicaid to transition out of nursing homes and live successfully in the community with ongoing supports. Clients served will be in Jefferson, Lewis, and St. Lawrence Counties. The average caseload is 12-15 clients per month.

Transition Services :

The Open Doors project is a statewide transition program aimed at helping Medicaid eligible individuals who want to leave the nursing home and return to the community. Through 'Open Doors,' the employee will help link the participant to services in the community that will help them live successfully including meals on wheels, homecare, emergency alert buttons, durable medical equipment, home modifications, and more. The staff member will complete Open Doors enrollment in-person at Nursing homes, screen for eligibility, obtain documentation showing the participant may be able to live successfully in the community, and will help develop a person centered plan with the Social Worker to help with discharge. The staff member will assist the participants with applications for services in the community, help the participant collect eligibility documents, help the participant navigate assessments for homecare, and will continue to meet with the participant frequently while working on obtaining services.

Housing Services:

The staff member will also help the same participants search for housing and complete applications for housing subsidies and transition funds. This includes intake and eligibility screening, collecting documentation to enroll in the Olmstead Housing Subsidy (OHS) Program, helping the participant search for apartments, helping the participant apply for subsidized and/or senior housing, and communicating with landlords about the participant. When housing is located, the staff member will complete a review of the apartment to assure it meets required standards of habitability. If the participant is eligible for an ongoing subsidy, the staff member will help calculate the participant's rental contribution and will collect ongoing rental information to maintain the subsidy. The staff member may also help the client pick out furnishings and assure their delivery. The staff member will also coordinate obtaining items the participant may be eligible for through the Open Doors program at discharge, such as a food card and welcome home package. Additionally, participants who have already been discharged from nursing homes but are now homeless or at-risk may be eligible for an additional program called Rapid Transition Housing Program (RTHP). The staff member will act as their housing specialist under this program to help them locate permanent housing, obtain furnishings funded through the program, and maintain an ongoing rental subsidy. Not all clients served with transition services will need housing support if their goal is to return to their own homes.

Independent Living Services:

Once living in the community, the staff member will develop an independent living plan to help the participant remain in the community and will check in with the participant routinely. These services are person-centered and will be tailored to the client. This can include, but is not limited to, helping the participant apply for benefits such as HEAP, making referrals to Care Managers, linking to NY Connects or Office for the Aging, independent living skills training such as how to use a cell phone to call for help when needed, and helping the client troubleshoot issues as they arise, and more.

Overall Responsibilities:

Travel throughout Jefferson, Lewis, and St. Lawrence Counties to provide face-to-face visits with consumers who want to leave the nursing home. Much of the follow up with the participant is via phone. Travel is essential to this program and required. Provide outreach on the program services including writing introduction letters to Nursing Homes, conducting presentations to nursing home staff and social workers, attending health fairs and community events to distribute flyers. Flyers can also be posted throughout the community. Outreach activities will be tracked and recorded. Work in collaboration with the lead coordinator of the project, NYAIL, long term care facilities, individuals, and family members to ensure the transition process is coordinated for each participant. Provide unbiased information, linkages, referrals, and advocacy on behalf of the participant. Assist the participant to develop a person-centered plan with their social workers. Assist the participant to apply for various programs including lifeline, SNAP, housing subsidies, low-income/senior housing, homecare programs, and more. Assist the participant to locate housing of their preference, communicate with landlords on the client's behalf to explain the Olmstead Housing Subsidy program, obtain documentation, and help calculate the rental subsidy amount the client is eligible for. Assist the participant to obtain household goods and supplies to assist with a successful transition, if applicable. Assist the participant navigate the home healthcare system to find a suitable program to meet their needs. This includes attending their homecare assessments, when applicable. Follow up routinely with the participant after discharge and develop goals and plans of services they may need to remain in the community. Document all interactions with the client or on behalf of the client timely in online databases. Maintain strong organization of client records, deadlines, and timelines.

REQUIREMENTS

All education and work histories will be considered Excellent oral and written communication skills Experience with public speaking a plus

+ Must have strong computer skills and experience with Microsoft Office programs; have the ability to learn new programs and databases; have proficiency using a computer/fax/copier machine. Ability to interact professionally with consumers who face various barriers, including disabilities. Respectful and comfortable with people from different backgrounds Act in a professional demeanor at all times. Ability to work independently and manage multiple tasks Must possess a valid driver's license, reliable vehicle, and vehicle insurance; must be willing to travel throughout the entire service area.

SUPERVISION

This position does not act in a supervisory manner The staff will report to the NYAIL leads, the Deputy Director of MILC, and the Executive Director of MILC. The staff member will have multiple individuals to seek guidance from. The staff member will have routine meetings to review caseload, develop strategies for the participant, and troubleshoot barriers.

BENEFITS AND PAY

Full Time Position, 40hpw, non-exempt Monday-Friday 9AM-5PM. These hours will not be reduced even during slow periods. 30 min paid lunch and x2 15 min paid breaks daily, so the workweek is 35hpw $19.25 per hour Generous Benefit Package includes: 11 paid Holidays 80 paid hours sick time 80 paid hours vacation annually. Years 1-4 are 80 hrs per year, Year 5 increases to 120 hours, and every year after that the employee is provided with an addition 8 hours of vacation time (ie: Year 6 = 128, Year 7 = 136, and so forth) 32 paid hours personal time Paid bereavement time Paid lunch and breaks Paid Snow Days & Snow Delays (based on local school closures) Paid jury duty leave 403(b) retirement Plan with an employer 3% contribution after a vested period with the agency Excellent health insurance for the employee! Single health coverage at only $60.00 a month premium. MILC does not offer Family health insurance, but we do offer Family Dental/Vision for an additional cost. MILC provides an HRA that pays 75% of the medical copays/coinsurance costs. 5.00 a month for dental + vision for the employee.
