$68,000 to $75,000 (Exempt) Reports to: Director of Case Management SCS has a hybrid work model with three days in office and two days remote. About the Role Sunnyside Community Services is seeking an experienced and dedicated Case Management Supervisor to lead a team supporting older adults to live safely, independently, and with dignity in their homes. As one of four Supervisors, you will oversee a team of approximately five Case Managers serving older adults across Queens Community Districts 1, 2, 3, and 5. You will provide guidance and clinical support while ensuring high-quality services, timely documentation, and compliance with NYC Aging standards. Key Responsibilities
- Provide weekly supervision, coaching, and support to approximately five Case Managers, ensuring quality service delivery and adherence to program and agency standards.
- Review case records and provide guidance to ensure documentation and services meet NYC Aging requirements and established timelines.
- Complete client intakes, assign cases, and monitor home care and home-delivered meal waitlists.
- Support Case Managers in advocating for clients and connecting them to benefits, entitlements, mental health and medical care, and other community resources.
- Maintain communication with clients, caregivers, and families when changes in a client's needs or circumstances require additional intervention.
- Coordinate referrals to emergency and specialized services, including mental health services, mobile crisis teams, elder abuse resources, and Adult Protective Services, when appropriate.
- Conduct in-home assessments and provide direct case support as needed.
- Partner with the Program Director to address operational challenges, support staff development, and maintain strong program performance.
- Maintain current knowledge of community resources and relationships with community and government agencies.
- Conduct community outreach and develop service linkages to help connect older adults with needed supports.
- Maintain required program records and administrative documentation, including staff training, timesheets, and intake data.
- Participate in intake coverage, trainings, conferences, and other program activities as needed.
- Maintain client confidentiality and communicate professionally and effectively with clients, families, staff, and partner agencies.
- Other duties as assigned by the program Director.

Office setting. Field visits with potential exposure to client illnesses.
Requirements:
- MSW and two years supervisory experience in Human Services Field
- Knowledge of Microsoft word, Excel, Outlook, PowerPoint
- Excellent verbal and written communication skills and computer literacy
- Solid organizational skills
- Good interpersonal skills with an ability to establish rapport with staff, clients, and other community and government agencies
- Must enjoy working with older adults.
- LMSW with SIFI certification a plus.
