Position Overview:

The Case Manager plays a vital role in supporting folks navigating early sobriety. This position focuses on providing individualized assistance with health, legal, and stability-related challenges, empowering clients to rebuild their lives and achieve sustained recovery. Initiative and the ability to be self-led is a must. Very few hours of your week will be pre-scheduled. A requirement of 25 billable hours per week means that you will need to create opportunities for case management where you see the need. A large portion of your role is finding the gap and filling it. Strong organizational skills are imperative. You will be responsible to keep track of client's who have and have not received case management and ensure that all clients are utilizing case management support.

Key Responsibilities:

Serve as an advocate for clients in accessing health, legal, and social services. Collaborate with clients to create individualized plans for addressing stability-related needs, such as housing, employment, and education. Monitor client adherence to treatment plans and facilitate communication between healthcare providers as needed. Provide referrals and correspondence to ensure continuity of care and transitions out of our program. Support clients in maintaining compliance with probation or parole requirements. Connect clients with resources for housing, food, transportation, and other essential needs.

•Transportation in your personal vehicle is a part of the role. Assist with job placement, vocational training, and educational opportunities. Provide support with financial planning, including budgeting and managing expenses. Maintain accurate and timely records of client interactions, progress, and outcomes. Prepare reports for internal use and external compliance as required. Ensure all documentation adheres to confidentiality and professional standards. Attend team meetings to discuss client progress and share insights.

Qualifications:

Current certification as Certified Chemical Dependency Counselor Assistant (CDCA) or greater. Valid driver's license and good driving record. Minimum of 2 years of experience in case management, substance use treatment, or a related role. Strong knowledge of recovery resources and community based connections Excellent communication, organizational, and problem-solving skills. Ability to build rapport with diverse populations and maintain professional boundaries.

Preferred Qualifications:

Familiarity with the 12-step recovery model.

Work Environment:

This is a salaried position with a flexible schedule. Most of your hours will be first shift but depending on the needs of the clients, it may include evening or weekend availability, as needed. This position is primarily residential-based. You will be working in our sober livings, directly with clients in their homes, not in an office setting.
