is seeking a dedicated and compassionate Case Manager to join the Safe Landing Youth Shelter team. Safe Landing provides short-term crisis shelter and support to youth ages 11-17 who are experiencing family conflict, crisis, or other challenging circumstances. The Case Manager plays an important role in helping youth navigate their stay at Safe Landing by providing support, coordinating services, maintaining accurate documentation, and helping connect youth and families with appropriate resources.
Schedule:
12 pm
  • 8 pm M-F and Every other Saturday 9 am
  • 5 pm or 12 pm
  • 8 pm: Off day on opposite weeks is negotiable.
What You'll Do Provide case management services to youth residing at Safe Landing. Complete and assist with intakes, discharges, and required documentation for youth entering and leaving the program. Ensure intake and case management documentation is complete, accurate, and properly signed. Coordinate appointments, referrals, and other services for youth and families. Maintain accurate case notes, youth records, logs, and other required documentation. Enter and maintain information in Celerity, RTIS, and other program systems as required. Communicate with counselors, direct care staff, families, schools, courts, Children Services, and community providers as appropriate. Help ensure continuity of services between shifts and between Safe Landing and the Respite Care Program. Facilitate or assist with youth groups and life-skills activities . Assist with planning and coordinating community outings and activities that promote positive youth engagement. Help youth identify strengths, goals, resources, and supports that can assist them during and after their stay. Advocate for youth while maintaining appropriate professional boundaries and confidentiality. Assist with crisis situations and provide support in a trauma-informed, youth-centered manner. Maintain awareness of program policies, licensing requirements, and documentation standards. Provide support to the shelter team and assist with other duties necessary to maintain safe and effective shelter operations. What We're Looking For The ideal candidate is someone who: Enjoys working with adolescents and families. Is compassionate, patient, dependable, and able to remain calm during challenging situations. Understands the importance of trauma-informed care and respectful communication. Has strong organizational and documentation skills. Can work independently while also being an effective member of a multidisciplinary team. Communicates effectively with youth, families, coworkers, and community partners. Is comfortable managing multiple responsibilities and changing priorities. Understands the importance of confidentiality and professional boundaries. Is willing to learn program procedures, documentation systems, and community resources. Qualifications Bachelor's degree in Social Work, Psychology, Human Services, Sociology, or a related field preferred. Experience working with youth, families, behavioral health, social services, or a residential program preferred. Case management or social services experience preferred. Must be able to successfully complete required background checks. Must have reliable transportation and the ability to travel locally when required. Must be able to meet all applicable agency and licensing requirements. Why Work at Safe Landing?
