Guided by our mission to "Love thy neighbor as thyself," we offer hope to those seeking food and basic needs, housing stability, and immigration services. We are seeking a Case Manager to join our Supportive Services team within our Long-Term Rental Assistance (LTRA) program . This program supports unhoused individuals and families with finding and maintaining housing, coordinating rental assistance, and building long-term housing stability and self-sufficiency.
POSITION SUMMARY
The Case Manager provides client-centered case management services to individuals and families enrolled in CCS programs. Working collaboratively with participants, community partners, and Agency staff, the Case Manager assesses needs, develops individualized service plans, connects participants to resources, and supports progress toward stability, self-sufficiency, and long-term success. This is an in-person position and is not remote.
SCHEDULE & COMPENSATION
Full-time, 40 hours per week Monday-Friday, with possible after-hours work for special events Non-exempt $21.50 per hour $22.50 per hour for bilingual English/Spanish employees who successfully complete CCS bilingual testing
KEY RESPONSIBILITIES
Receive, review, and respond to participant referrals. Assess participant strengths, needs, barriers, and risk factors. Determine program eligibility according to established guidelines. Develop and maintain individualized service plans. Assist participants in locating and maintaining housing. Conduct housing stability assessments. Conduct participant meetings, home visits, office visits, and community-based contacts as appropriate. Connect participants with internal and community resources. Work collaboratively with clients to promote engagement and progress toward self-sufficiency goals. Maintain timely data entry and program documentation. Collaborate with landlords, property managers, service providers, and other community partners. Monitor program-related eligibility requirements. Support housing retention and landlord mediation efforts. Monitor participant progress and revise service plans as needed. Develop discharge or transition plans that promote long-term stability, as applicable. Participate in staff meetings, trainings, community partner meetings, outreach activities, and other assigned projects.
MINIMUM QUALIFICATIONS
Associate degree in Human Services, Social Work, Psychology, Sociology, or a related field; OR an equivalent combination of education and experience. Two years of experience providing case management, social services, housing services, refugee services, family support services, or related human services work. Knowledge of case management principles and practices. Ability to assess needs and develop service plans. Strong interpersonal and relationship-building skills. Ability to maintain professional boundaries. Ability to work effectively with diverse populations. Strong organizational and time-management skills. Ability to work independently and collaboratively. Proficiency with Microsoft Office and electronic record systems. Valid Oregon Driver License, reliable vehicle, and automobile insurance.
PREFERRED QUALIFICATIONS
Experience with housing programs, homelessness services, HMIS, and rental assistance programs.
