Transfer Lateral Reassignment Voluntary Demotion Promotion Job description : This position provides case management for assigned caseload•assisting individuals with receiving needed services to carry out their Individual Support Plan and to provide on-going monitoring of client plan and any new client situations that may arise.
Essential Functions of Job:
Develops Individual Support Plan (ISP) for caseload and coordinates services as set forth in ISP. Completes PUNS process with each consumer on caseload. Maintains consumer progress notes/records. Acts as advocate for clients in school, community or other needs areas. Adheres to regulations/policies that apply to the provision of Support Coordination services.
Additional Primary Job Duties:
Maintains contact with consumers as minimally required or more frequently. Enters client data in HCSIS for maintenance of records. Conducts phone contacts with consumers, families, physicians or others in regard to consumer needs and services. Assists consumers with completion of necessary paperwork. Facilitates meetings with consumers, family, providers, etc. as required. Completes requests and referrals for evaluations, services, etc. as required. Works with individuals in need of crisis intervention. Counsels families, individuals and providers. Interview clients to obtain social histories. Provides comprehensive information about agency programs. Other Duties of the
Job:
Attends meetings, trainings and in-services as required by regulation. Educates and encourages the general public to support the IDD population. Performs other job related duties and case management and support services as required. Travels frequently to perform essential functions of job, to homes, providers etc. Some evening work (attendance at appointments, meetings) is involved in the performance of the job. Operates computer and software programs and other office equipment as necessary to carry out job duties.
MET's:
Six months of experience as a Human Services Case Manager; OR A bachelor's degree with a social welfare major; OR A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social services agency; OR Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
