Human Services, Inc . (HSI) is a recovery driven community services provider that has been serving the Chester County community for more than 50 years. HSI is committed to providing a variety of vital homeless support services to those in need. The Case Manager is a full-time exempt position available in the Homeless Support Services Department at HSI located in Thorndale, PA. HSI currently has one Case Manager positions available: Critical Time Intervention Specialist /Homeless Case Manager. The Critical Time Intervention Specialist/Homeless Case Manager is responsible for the following:
Essential Job Functions:
Provide street/shelter outreach to potential and current participants Assist people receiving services in making critical transitions in their lives. Assist and facilitate in creating linkages to long term community supports between people receiving services and community service agencies. Assess, evaluate and document strengths and needs of each person on the Specialist's caseload. Work collaboratively with other Critical Time Intervention team members to improves experiences for people receiving services. Facilitate meaningful community engagements with participants in the Critical Time intervention program Mediate and advocate on behalf of participants in the program to help them obtain and maintain permanent housing, health care, other sustaining benefits, entitlements, and transportation. Document each engagement with the people on the Specialist's caseload. Participate in Clinical Consultation process and discuss strategies to improve services. Travel throughout Chester County including Oxford, Nottingham, Phoenixville etc. is required using a shared company vehicle.
The following clearances are required:
FBI/Child Abuse History/PA Criminal History/PA Driver's License. Employees must maintain a valid driver's license and current automobile insurance.

Minimum Qualifications:
Bachelor's degree in social work, human services or related field.
Must be able to perform duties at a fast pace.
Have strong organizational and time management skills.
Must possess the ability to communicate effectively both orally and in writing.
Establish positive working relationships with staff, community partners and the individuals you work with.
Relate positively with colleagues and representatives of other agencies.
Have the ability to recognize and be sensitive to cultural and ethnic differences.
Must be able to safely perform essential job functions with or without reasonable accommodations.
