Job Description

Position Description:
CASE MANAGER POSITION SUMMARY
This position is responsible for day-to-day direct service operations including scheduling of DCWs, payroll and related dispute intake or resolutions. This position is cross trained for HR Supervisor & Client Service Supervisor Position.
RESPONSIBLE TO
Regional Program Director
ACTIVITIES
1. Handles all incoming calls to the office as it relates to new schedules or operations. 2. Works with Client Service Supervisor regarding initial DCW assignment in order to comply with schedule and level of competency needs. 3. Handles DCW ongoing scheduling including call-offs and DCW changes. 4. Reviews Electronic Visit Verification (EVV) addressing discrepancies including verification with client and DCW instruction as needed. 5. Maintains communication with MCOs, clients, DCWs and office staff. 6. Completes the "missed service" report. 7. Processes DCW and office-based staff payroll-related documents to payroll service. 8. Confirms accuracy of payroll prior to sending to administrator for processing. 9. Processes payroll to payroll service. 10. Serves as "back-up" for incident reports. 11. Must be available for on-call coverage assistance and direct care services.

Maintains a professional standard of conduct, projecting a positive image of the agency always. B. Complies with infection prevention/control, risk management, and safety policies and procedures. C. Adheres to all policies and practices of the agency to maintain Service Standards. D. Ensures the confidentiality of client and personnel communication including written, verbal, and electronic information, adhering to HIPAA and related regulations. E. Works with consumers and employees in a caring, therapeutic, culturally sensitive professional manner. F. Respects consumer control and the independent living philosophy.
QUALIFICATIONS A.
High school graduate or GED. B. Fluent in Spanish and English (reading, writing and speaking). C. Two years of experience in health care settings. Care coordination or direct service experience preferred. C. Computer skills in Word and Excel. D. Availability for on-call coverage, evenings and weekends. E. Interpersonal communication, time management and organizational skills. Evidence of leadership and/or supervisory activities. F. Knowledge of office and/or personnel management services, including needs identification, and meeting quality standards. G. When required to travel from one assignment to another on the same day, a reliable, independent means of transportation is required. Drivers must have a valid driver's license and car insurance limits adhering to the limits established in the personnel policies. H. Obtains and retains clear criminal records check, FBI, child abuse and required clearances such as
OIG, EPLS/SAM.
