Position Overview:

Under the supervision of the Community Youth Service (CYS) Supervisor, and in collaboration with area school districts and governmental entities, the Youth Services/CYS Case Manager provides school-based crisis intervention/prevention, short-term case management, assessment, service coordination, and referral services for at-risk students and their families. This position supports safety and stability, links clients to essential resources, maintains confidential database documentation, and provides crisis response and debriefing support following traumatic events.

Key Duties & Responsibilities Crisis Intervention & Counseling:

Provide school-based crisis counseling, short-term problem solving, safety planning, and follow-up services to at-risk youth and families; serve as a member of the district crisis response team to conduct debriefings as needed.

Assessments & Service Coordination:

Conduct screenings and needs assessments to develop short-term care plans; coordinate multi-system services in collaboration with school personnel and external partners.

Community Resources & Referrals:

Connect youth and families to community-based and school-based resources; follow up to support engagement and remove service barriers; network to expand community partner awareness.

Documentation & Outreach:

Prepare and maintain comprehensive, timely case documentation in the agency database according to confidentiality standards; deliver presentations and promote CYS services to school staff and community members.

Qualifications & Requirements Education:

Bachelor's degree in Psychology, Sociology, Social Work, Criminal Justice, Counseling, or a closely related field required.

Experience:

Minimum of one (1) year of paid experience providing crisis counseling and case management services to youth and families required.

Experience using Microsoft Outlook, Word, Excel, PowerPoint, and dedicated Case Management Software.

Licensure & Transportation:

Valid Class C Texas Drivers License required.

Reliable personal transportation with auto liability insurance required.
