Job Description

Mission Granbury is seeking motivated, organized, dependable applicant to fill a full-time Direct Services Case Manager position at the Ada Carey Center Shelter and non-residential case management at the Mission Granbury Resource Center . The job could include evening, weekend or holiday hours. Duties include assisting with crisis intervention, answering the 24-hour crisis line, safety planning, advocacy and other supportive services to clients of domestic violence and/or sexual assault. Must have reliable transportation, the ability to pass a background check and be able to lift 25 lbs. The position starts at $35,000 to $42,500.00 annually. No phone calls please. GENERAL DESCRIPTION
  • Identify, develop, and maintain resources with relevant service providers, community organizations, and other entities with a goal of moving victims of domestic violence and sexual assault from crisis to self-sufficiency.
  • Provides case management and other direct support to residential & non-residential victims including assistance with safety planning, temporary emergency housing, crisis intervention, legal referral, document replacement, victim advocacy, immediate clothing/food assistance, financial assistance, transportation, accompaniment (as requested legal, medical, etc.
), and follow-up services. Conduct assessment of immediate client needs and connect them to appropriate resources to meet those needs. Complete Intake Assessment with new clients in a timely manner.
  • Assist with answering the 24-hour crisis hotline completing victim intakes by phone and/or in-person.
  • Provide referrals and educate clients about available resources that are specifically related to the client's needs and situation. (healthcare, childcare/development, mental health, etc.)
  • Work with public and private systems to advocate for the best interests of clients as needed to help client access necessary services.
  • Collect, review, and input data efficiently into the software system for residential and non-residential clients. This will include maintaining complete and accurate client records that document services provided as outlined by Mission Granbury policies and procedures.
  • Develop individual and specifically tailored case plans for clients that include goals, objectives, tasks, and timeline that is realistic and attainable for the client to achieve safety and stability.
  • Conducts weekly case management meetings with clients to review case plan, track progress, co-create goals, monitor progress, provide resource referrals to achieve positive outcomes, provide guidance and encouragement.
  • Evaluate and make recommendations regarding client services.

WORK EXPERIENCE AND SKILL REQUIREMENTS
Ability to organize workflow, respond to emergent situations, and note detail. Ability to maintain a high level of performance in an environment of frequent demands and changes. Ability to work independently as well as part of a team for cooperative problem solving. A flexible schedule is required as this is a 24/7 facility. Ability to communicate effectively, both orally and written. Must demonstrate experience with and sensitivity to varying values, perspectives, cultural, ethnic, social backgrounds, and gender identity. Basic computer skills. Driver's License Required
EDUCATION REQUIREMENTS
Some college preferred. High School Diploma or GED required. Minimum of two years' experience in a victim assistance or family violence shelter program.
