The Eagle Mountain Family Clinic provides a variety of therapeutic services for clients affected by trauma and/or mental illness. As a Case Manager, you'll help clients access community resources, coordinate services, and connect with the support they need while working alongside a collaborative multidisciplinary team to help clients build stability, confidence, and progress toward their personal goals.

What You'll Do as a Case Manager Conduct client assessments and help create individualized care plans. Coordinate services and collaborate with other providers to support client needs. Offer ongoing support, guidance, and follow-through with clients. Connect clients and families with community resources and supportive services. Work closely with a supportive, multidisciplinary team. Help clients navigate appointments, services, and other resources. Build positive relationships with clients and families through compassionate, client-centered support. Help clients build stability, confidence, and progress toward personal goals. Maintain accurate documentation, case notes, and client records. Qualifications Applicants must meet ONE of the following:
Note:
Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire. Bachelor's degree with Social Service Worker (SSW) license Bachelor's degree in any field with willingness to pursue SSW licensure
Additional Requirements:
