Sol Recovery Community, Inc. is seeking a dedicated Case Manager to join our team! This role is essential in supporting clients on their journey to recovery by connecting them with resources, coordinating referrals, and ensuring a seamless intake process. Additionally, this position includes on-site case management services through our supported living program.
Key Responsibilities:
Guide clients and families through the intake and referral process. Ensure completion of intake documentation. Develop and maintain relationships with community partners. Meet with clients individually to assess and address their needs. Provide on-site case management services for supported living clients. Connect clients to vital community resources and higher levels of care. Coordinate referrals to residential and detox facilities. Observe and document urinalysis testing as required. Maintain detailed case management notes in compliance with SRC policies. Collaborate with the clinical treatment team and attend weekly case staff meetings. Uphold confidentiality and ethical standards. Assist with community outreach and presentations. Perform other job-related duties as assigned.
Requirements:
Must be at least 18 years of age. Social Service Worker license (SSW) or Case Management Certification (or willingness to obtain). If not already certified, the case manager will be supported in completing Domestic Violence (DV) Advocacy Training to ensure they are equipped to work with this population. Clean criminal record (no felonies or related misdemeanors per licensing standards). Must complete a TB screening and test only if indicated. Ability to pass post-offer and random drug screenings. Strong computer skills, including Google Workspace, data management, and email. Reliable and punctual with excellent communication skills. Ability to read, write, and communicate fluently in English.
Work Environment:
Primarily an office-based role with some on-site case management responsibilities. Requires the ability to sit for extended periods, communicate effectively, and visually observe UA testing.
