The successful candidate will be responsible for providing support to clients, assessing their needs, and developing individualized plans to meet those needs. The Case Manager will also coordinate with other service providers, provide referrals to community resources, and ensure that clients receive the necessary services. Responsibilities:

Assess client needs and develop individualized service plans. Provide case management services to clients, including referrals to community resources. Maintain accurate and up-to-date client records. Conduct home visits and other assessments as needed. Monitor client progress and make necessary adjustments to service plans. Provide crisis intervention services when needed. Develop relationships with community resources to ensure quality services for clients. On-call one weekend every seven weeks

$46,000.00 - $55,000.00 per year
