Come join our dynamic team with a passion for providing services and support for individuals with mental and behavioral health needs. We are looking for an experienced Case Manager to join our team. The successful candidate will be responsible for providing support to clients, assessing their needs, and developing individualized plans to meet those needs. The Case Manager will also coordinate with other service providers, provide referrals to community resources, and ensure that clients receive the necessary services. This is an excellent opportunity for a passionate and dedicated individual to make a difference in the lives of those we serve.
Responsibilities:
Assess client needs and develop individualized service plans. Provide case management services to clients, including referrals to community resources. Maintain accurate and up-to-date client records. Conduct home visits and other assessments as needed. Monitor client progress and make necessary adjustments to service plans. Provide crisis intervention services when needed. Develop relationships with community resources to ensure quality services for clients. On-call one weekend every seven weeks

$46,000.00 - $55,000.00 per year
