We are seeking a compassionate, dependable, and organized Case Manager to join our substance abuse treatment team. The Case Manager will work directly with clients throughout treatment, helping them address barriers to recovery and connect with the services and resources they need to build a stable life in recovery. Key Responsibilities Complete case management assessments and identify client needs, strengths, and barriers to recovery Develop and monitor individualized case management goals and service plans Help clients obtain housing, transportation, employment, healthcare, identification, benefits, and other community resources Coordinate referrals and services with community providers and treatment professionals Support clients with discharge planning and successful transitions to continuing care Assist clients in developing recovery-supportive resources and connections Advocate for clients and help them navigate healthcare and social-service systems Maintain accurate and timely case notes and required documentation Communicate and collaborate with counselors, peer support staff, medical providers, and other members of the treatment team Maintain appropriate professional boundaries and client confidentiality Participate in treatment-team meetings, trainings, and other facility activities Provide services in a respectful, trauma-informed, recovery-oriented manner Qualifications Experience in substance use disorder treatment, behavioral health, case management, or social services preferred Knowledge of recovery resources and community-based services Strong communication, organization, and problem-solving skills Ability to work effectively with individuals experiencing substance use and related life challenges Compassionate, nonjudgmental, and client-centered approach Ability to maintain professional boundaries and confidentiality Bachelor's degree in human services, social work, psychology, counseling, or a related field required

Pay:

$45,000.00 - $55,000.00 per year
