Job Description

Full-Time $55,000-$60,000 Generous PTO Paid Holidays Health Care Cost Share Position Summary The Residence Case Manager works directly with residents living in our recovery residence, helping them develop individualized goals, connect with needed services, and build the skills necessary for long-term recovery, stability, and independence. Key Responsibilities
  • Develop and monitor individualized recovery and service plans.
  • Meet regularly with residents to provide support, accountability, and encouragement.
  • Coordinate healthcare, behavioral health, substance use treatment, employment, education, transportation, benefits, housing, and other community services.
  • Help residents navigate systems, overcome barriers, and develop self-advocacy skills.
  • Respond appropriately to crises and assist with relapse response planning.
  • Maintain accurate, timely, and confidential documentation.
  • Collaborate with residential staff, clinical providers, peer support specialists, employers, healthcare providers, families, and community partners. Qualifications
  • Experience in case management, behavioral health, recovery services, social services, or a related field preferred.
  • Excellent communication, organization, and documentation skills.
  • Ability to build trusting relationships while maintaining healthy professional boundaries.
  • Strong problem-solving skills and ability to coordinate multiple services.
  • Compassionate, dependable, and committed to supporting individuals in recovery.

Pay:
$23.00 - $25.00 per hour
