Working from the New Hope Resource Center, this position builds trusting relationships with participants, co-creates participant-directed service plans, connects people to housing and community resources, and documents services accurately in HMIS. Success in this role looks like consistent engagement, strong provider relationships, and measurable movement toward each participant's self-identified goals.
Key Responsibilities:
Engagement
- Conduct proactive and professional engagement to build rapport with people living unsheltered.
- Apply harm reduction, Motivational Interviewing, Trauma-Informed Care, and Housing First principles in every interaction.
- Offer non-judgmental, unconditional, low-barrier support, and meet participants where they are, without requiring sobriety, treatment participation, or program compliance as a condition of service.
- Distribute basic-need supplies and conduct wellness checks as directed by program protocols. Case Management and Service Planning
- Meet regularly with participants to develop and update participant-directed service plans, set goals, and navigate complex housing, benefit, and health systems.
- Tailor your approach to each person's strengths, personality, culture, language, identity, and interests; focus on creative solutions to barriers rather than on deficits.
- Complete intakes, assessments, and applications with participants, and accompany them to appointments when helpful.
- Coordinate with the local Coordinated Entry provider so participants are screened and prioritized based on vulnerability.
- Provide follow-up with participants transitioning into housing or another program to support stability and ensure outcomes are recorded accurately.
- Transport participants to appointments, housing tours, and basic needs in a personal or agency vehicle as approved. Documentation and Data
- Enter participant data into HMIS accurately and within [2 business days] of each contact.
- Maintain complete, professional, and timely case notes and participant files that meet funder, grant, and confidentiality requirements.
- Contribute to monthly program reporting and outcome tracking as requested. Community Partnership and Advocacy
- Maintain current working knowledge of local services and resources, including substance use and mental health assessment and treatment, recovery supports such as AA/NA, shelter and housing programs, veterans' services, benefits and financial education, clothing and hygiene resources, medical and dental care, disability services, and education and employment programs.
- Build and sustain relationships with service providers to make warm, effective referrals, and advocate for participants to receive appropriate services.
- Represent the organization professionally at community meetings and provider coalitions as assigned. Team and Organizational Contribution
- Participate in team meetings, individual supervision, case consultation, and required training.
- Work effectively both independently in the field and as part of a collaborative team.
- Perform other duties that support the organization's mission and operational needs. Minimum Qualifications
- One year of experience in case management, outreach, peer support, behavioral health, housing, or a closely related human services role. Equivalent lived experience or relevant volunteer experience will be considered.
- Ability to communicate respectfully and effectively with people in crisis, in writing and in person.
- Basic computer skills, including email, Microsoft 365, and comfort learning a database such as HMIS.
- Ability to maintain professional boundaries and confidentiality, including HIPAA and HMIS privacy standards.

- Demonstrated ability to deliver culturally responsive services and work effectively across cultures, identities, and life experiences.
- Reliable transportation for field-based work. A valid driver's license, current auto insurance, and a driving record acceptable under the organization's driving policy are required for positions that transport participants; a current driving abstract must be provided.
- Must pass required background clearances before an offer of employment is final. A criminal record does not automatically disqualify you; we review each situation individually and in line with [applicable Washington State fair-chance requirements]. Preferred Qualifications
- Associate or bachelor's degree in social work, human services, psychology, or a related field.
- Direct experience with street outreach, Safe Parking, or mobile crisis response.
- Lived experience of homelessness, housing instability, recovery, or the systems our participants navigate.
- HMIS, Coordinated Entry, Mental Health Professional, Peer Counselor, CDP/CDPT, Naloxone, CPR/First Aid, or de-escalation training or certification.
- Bilingual in [Spanish / Russian / Ukrainian / Marshallese / other priority language]. Physical and Working Conditions
- Field-based work in all weather conditions, including walking over uneven ground, into encampments, and to locations vehicles cannot reach.
- Ability to walk or stand for extended periods and to lift and carry up to [25] pounds of supplies occasionally.
- Frequent driving in Pierce County; regular use of a computer and phone.
- Exposure to secondary traumatic stress, unsanitary conditions, and occasionally unpredictable or unsafe environments. The organization provides safety protocols, [paired outreach], training, and [supervision, EAP, and reflective support] to mitigate these risks.
