Overview The Pierce County Alliance has one FTE Peer Pathfinder position. The Peer Pathfinder will work with participants who are exiting correctional facilities who have either a serious mental illness, a substance use disorder, or co-occurring conditions. The Peer Pathfinder will support the jail transition team with release planning efforts for the participant to reengage in the community after release and attempt to engage participants in planning their therapeutic care. These supports may include spending time establishing social support, helping with independent living skills, developing coping skills, and community adjustment skills. The Peer Pathfinder is not a case manager, discharge planner, or a crisis worker. However, the Peer Pathfinder can bring the participants' perspective into the provision of those services. Responsibilities Responding to participants' various needs via in-person, phone, and telehealth communications Keeping communication open with participants SUD Counselor and Mental Health Therapist Participate in Peer Pathfinder training events by Carelon. Complete the current Carelon Peer Pathfinder report/log, submit log to Carelon via secured email. Complete the Intersections of Behavioral Health and the Law training Assist the participant in developing a crisis plan with the behavioral health service agency. Assist the participant in developing a life structure, including skills for daily living such as visits to coffee shops, use of local transportation, opening a bank account, working effectively with a payee if needed, understand benefits, budget planning, shopping and meal preparation, access leisure activities, find a church or faith home, attain, and maintain housing etc. Support the participant in developing skills to schedule, track and attend appointments with providers. Other duties as assigned. Requirements Must be 21 or older. High School Diploma, GED, or Higher Education. Must have a Certified Peer Support Specialist certification If offered employment you must pass criminal history record check through DSHS as applicable. If offered employment you must pass a Washington State Patrol and FBI background check as applicable. Must have a valid Washington State Driver's License and ability to travel. Must have the agency's minimum car insurance liability limits. Must pass a monitored urinalysis testing. Must have no history of alcohol or other drug misuse for a period of three years immediately prior to the time of employment. Experience Demonstrated passion for supporting others' growth and development. Excellent communication skills with the ability to listen actively and provide constructive feedback. Experience in mentoring, coaching, or community outreach is preferred. Ability to work independently while collaborating effectively within a team environment. Strong organizational skills to manage multiple activities and maintain accurate records. Familiarity with resources related to education, employment, mental health, or social services is a plus.
