The Case Manager will play a critical role in managing client cases, facilitating access to necessary services, and advocating for client needs within the community. Responsibilities Conduct comprehensive assessments of client needs and create tailored service plans. Collaborate with healthcare providers, social workers, and other professionals to coordinate client care and services. Monitor and evaluate the effectiveness of the service plan, making adjustments as necessary. Provide crisis intervention as needed and advocate for clients within the system to secure necessary resources. Maintain accurate and confidential client records and report on case progress and outcomes. Engage in community outreach activities to educate the public and identify resources for client support.
