WISCS.org is seeking a compassionate and dedicated Case Manager to join our mission-driven team focused on enhancing community services and public welfare. Our ideal candidate will uphold our commitment to delivering innovative social support solutions, drive initiatives that improve social justice, and sustain the quality and effectiveness of our services. The Case Manager will play a critical role in managing client cases, facilitating access to necessary services, and advocating for client needs within the community. Responsibilities Conduct comprehensive assessments of client needs and create tailored service plans. Collaborate with healthcare providers, social workers, and other professionals to coordinate client care and services. Monitor and evaluate the effectiveness of the service plan, making adjustments as necessary. Provide crisis intervention as needed and advocate for clients within the system to secure necessary resources. Maintain accurate and confidential client records and report on case progress and outcomes. Engage in community outreach activities to educate the public and identify resources for client support. Required Skills Strong interpersonal and communication skills Ability to manage multiple cases efficiently under pressure Proficiency in database management and documentation Understanding of local community resources and social services networks Crisis intervention skills
