Job Description
About us Colonial Club Senior Activity Center is located in Sun Prairie, WI. Our goal is to enhance the well being and independence of our older adults. The organization is a charitable, non-profit organization and serves all of NE Dane county. We have a family atomosphere within our organization.
Our work environment includes:
Relaxed atmosphere Flexible working hours Casual work attire On-the-job training ```Duties```- Provide case management services to older adults in need
- Conduct assessments and develop individualized service plans
- Coordinate and monitor the delivery of services to clients
- Collaborate with community resources to ensure clients' needs are met
- Advocate for clients' rights and access to services
- Maintain accurate and up-to-date case records ```Skills```
- Strong background in social work or related field
- Knowledge of older adult programs and related resources
- Familiarity with Medicare, Section 8, and other public assistance programs a plus
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
