Job Description

The Case Manager is responsible for the recruitment and retention of participants into the Transitional Jobs (TJ) program for Region 7 counties ( including Ashland, Burnett, Douglas, Price, Rusk, Sawyer, Washburn, et al), providing individualized case management and supportive services; development and implementation of individualized employment and training plans; establishment of SMART goals; coordination with partner agencies of needed services such as education, training, formal assessments, etc.; development of participant work sites and job coaching, and on-going follow-up and support. The Case Manager is the primary case manager and has the ultimate responsibility for the case. The Case Manager is responsible to meet individual performance goals. Ability to assess vocational skills and needs of defined service populations; knowledge of community resources and potential participants, positive work ethic, knowledge of business needs and the economy in the geographical area assigned; effective interpersonal and counseling skills; group facilitation and organizational skills; ability to keep accurate records/case noting and data entry; and ability to work independently, The majority of case management is completed remotely so employee can work from home. Travel within the region assigned is required when there is a need to meet a participant in person or to perform outreach for recruitment. Bachelors degree desired in Vocational Rehabilitation, Social Work, Psychology or related fields or 3-5 years of experience in related field. Criminal background check required. Full benefit package. $22.50 - $23.50 per hour
