Job Description

OVERALL RESPONSIBILITIES
The Lead Peer Navigator serves as the primary community engagement and navigation lead for prevention, education, outreach, treatment navigation, HIV prevention, overdose prevention, and recovery support initiatives. Utilizing a peer-based and recovery-oriented approach, the Lead Peer Navigator is responsible for coordinating outreach activities, facilitating community partnerships, supporting program implementation, and ensuring achievement of grant and program deliverables. This position provides direct navigation services to individuals at risk for substance use disorders, HIV/AIDS, and related health disparities while supporting outreach staff and peer navigators in community engagement efforts. The Lead Peer Navigator serves as a liaison between community members, healthcare organizations, behavioral health providers, educational institutions, social service agencies, and recovery support systems to improve awareness, increase access to services, reduce stigma, and strengthen linkage to care. The Lead Peer Navigator assists with program planning, quality improvement activities, data collection, reporting requirements, community education initiatives, and coordination of outreach strategies throughout designated service areas. The position represents Eggleston Youth Centers at community meetings, coalitions, public events, and collaborative partnerships.
DUTIES:
Leadership, Coordination & Program Support 1. Assist program leadership with implementation of grant and program activities. 2. Coordinate outreach schedules, community events, educational presentations, and engagement activities. 3. Support onboarding, training, and mentoring of Peer Navigators and outreach staff. 4. Monitor outreach activities to ensure alignment with grant objectives and program performance measures. 5. Identify service gaps, barriers to care, and emerging community needs and communicate findings to program leadership. 6. Assist with development of outreach plans, partnership strategies, and community engagement initiatives. 7. Support achievement of contractual deliverables, performance outcomes, and reporting requirements. 8. Participate in strategic planning, quality improvement initiatives, and program development activities. Community Outreach & Engagement 9. Conduct community outreach throughout Los Angeles County and designated Service Planning Areas. 10. Develop and maintain relationships with community-based organizations, schools, healthcare providers, treatment programs, faith-based organizations, and other stakeholders. 11. Participate in health fairs, community events, educational forums, coalition meetings, and resource fairs. 12. Promote awareness of substance use prevention, HIV prevention, treatment services, recovery resources, and available community supports. 13. Represent Eggleston Youth Centers as a professional ambassador within the community. 14. Assist with community assessment and identification of populations experiencing disparities and barriers to services. Prevention Education 15. Deliver educational presentations addressing substance use prevention, HIV/AIDS prevention, overdose awareness, recovery support, and behavioral health topics. 16. Provide education regarding opioid, stimulant, alcohol, and polysubstance use disorders. 17. Support implementation of evidence-based and evidence-informed prevention programs. 18. Participate in public awareness and stigma reduction campaigns. 19. Assist with development and distribution of educational materials and prevention resources. 20. Promote health equity, harm reduction, and recovery-oriented messaging. Treatment Navigation & Linkage Services 21. Provide screening, referral, and navigation support to individuals seeking behavioral health, substance use disorder treatment, HIV services, primary care, housing, employment, and other community resources. 22. Conduct warm handoffs and linkage-to-care activities with partnering service providers. 23. Assist individuals in overcoming barriers affecting treatment engagement and service access. 24. Follow up with participants to support service engagement and continuity of care. 25. Maintain current knowledge of local treatment providers, recovery resources, healthcare systems, and community support services. HIV Prevention & Health Education 26. Coordinate and support HIV prevention education activities. 27. Facilitate referrals for HIV, Hepatitis C, and other health screenings as appropriate. 28. Promote awareness regarding HIV prevention, risk reduction strategies, and available support services. 29. Collaborate with healthcare and community partners to improve linkage to prevention and treatment services. 30. Support community education initiatives targeting underserved and vulnerable populations. Overdose Prevention & Harm Reduction 31. Provide overdose prevention education and training. 32. Conduct naloxone education and distribution activities. 33. Educate participants regarding overdose recognition and response. 34. Support harm reduction initiatives designed to reduce substance-related harms within the community. 35. Maintain required documentation related to naloxone distribution and overdose prevention services. Peer Support & Recovery Services 36. Utilize lived experience, recovery principles, and strengths-based approaches when engaging individuals. 37. Provide peer support that promotes resilience, recovery capital, wellness, and community connectedness. 38. Assist individuals with goal development, problem-solving, and service access. 39. Model recovery-oriented and trauma-informed engagement practices. 40. Encourage participant self-advocacy and empowerment. Documentation, Reporting & Data Collection 41. Maintain accurate and timely documentation of outreach activities, educational encounters, referrals, linkage services, and participant contacts. 42. Assist with collection and reporting of grant outcome data and performance measures. 43. Monitor outreach and service delivery metrics to support program success. 44. Prepare reports, summaries, and participation logs as required. 45. Ensure compliance with agency, funding, and regulatory requirements. Collaboration & Team Participation 46. Participate in staff meetings, trainings, supervision, and quality improvement activities. 47. Collaborate with internal programs and external community partners. 48. Participate in advisory boards, coalitions, and stakeholder meetings. 49. Foster positive relationships with referral sources and partner organizations. 50. Perform related duties and special projects as assigned.
QUALIFICATIONS 51.
High School Diploma or GED required; Associate's or Bachelor's degree in Social Services, Public Health, Behavioral Health, Human Services, or related field preferred. 52. Certified Peer Support Specialist and/or Registered or Certified Substance Use Disorder Counselor preferred. 53. Minimum two (2) years of experience providing outreach, community engagement, peer support, prevention services, behavioral health services, or navigation services. 54. Experience coordinating community partnerships and outreach initiatives preferred. 55. Knowledge of substance use disorders, HIV prevention, recovery-oriented systems of care, harm reduction, and behavioral health services. 56. Knowledge of community resources, referral systems, and linkage-to-care processes. 57. Strong organizational, leadership, communication, and public speaking skills. 58. Ability to work independently while effectively supporting a multidisciplinary team. 59. Proficiency with Microsoft Office and electronic documentation systems. 60. Valid California Driver's License and insurable driving record.
PHYSICAL REQUIREMENTS 61.
Ability to travel throughout Los Angeles County. 62. Ability to stand, walk, and transport outreach materials up to 25 pounds. 63. Ability to conduct presentations and community events. 64. Ability to work occasional evenings and weekends based on program needs.
ORGANIZATION RELATIONSHIPS/INTERACTIONS
The Lead Peer Navigator works closely with the Clinical and Program Development Manager, Project Director, Peer Support Specialists, community partners, treatment providers, healthcare organizations, schools, social service agencies, and community stakeholders. This position serves as a lead community liaison responsible for strengthening partnerships, improving access to services, supporting peer navigator activities, and advancing agency prevention, treatment engagement, and recovery support initiatives.
Pay:
$24.00 - $26.00 per hour People with a criminal record are encouraged to apply
