Under the direction of the CHIP Partner and in partnership with the Alameda County Public Health Department (ACPHD), the Community Health Improvement Plan (CHIP) Partner Liaison is responsible for ensuring implementation advances through activities co-led by BayPLS. In collaboration with ACPHD, the Liaison translates strategic intent into coordinated action, ensuring that community-based CHIP implementation activities remain responsive, cohesive, and results-driven. The CHIP Liaison is selected in partnership with ACPHD and is a full-time employee of BayPLS. This position is dedicated solely to the CHIP partnership and its approved scope of work, deliverables, engagement activities, data requirements, and evaluation responsibilities.
THE ROLE
The Liaison plays a critical role in advancing CHIP implementation by working closely with ACPHD staff to translate CHIP goals, priorities, and requirements into actionable guidance for how BayPLS achieves agreed-upon deliverables. The Liaison provides continuity across the CHIP implementation lifecycle, preserves institutional knowledge, tracks progress, and helps ensure commitments remain aligned with approved scopes of work and evaluation expectations. By serving as BayPLS's primary point of contact with ACPHD and other CHIP partners, the Liaison maintains consistent, direct communication, ensures information flows smoothly, clarifies expectations, and addresses questions or concerns promptly.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Coordinate and participate in regular meetings with ACPHD staff and the other CHIP anchor partners. Identify, recommend, and support implementation of interventions aligned with the assigned CHIP priority and approved scope of work. Engage community stakeholders, partners, and residents in CHIP implementation activities and maintain productive community-level partnerships. Plan and co-host convenings, meetings, forums, outreach activities, or other events that contribute to CHIP implementation. Support policy analysis and policy development related to the assigned CHIP priority. Contribute to community health assessment activities and related planning efforts. Engage in best practices, knowledge development, and knowledge sharing regarding CHIP activities and evaluation practices. Collect, manage, validate, and submit data related to agreed-upon health improvement indicators, programming, deliverables, and performance measures. Ensure timely, accurate, and regular delivery of required data and documentation to ACPHD and contribute to evaluation efforts through quantitative and qualitative data, lessons learned, success stories, and other required reporting. Work directly with ACPHD and CHIP partners to execute contractual expectations and manage multiple CHIP workstreams and deliverables at the same time. Maintain organized project records and update internal databases, trackers, and shared documentation to support transparency, collaboration, and continuity. Provide timely information, documents, progress updates, and results to ACPHD, BayPLS leadership, and relevant CHIP stakeholders. Liaise between ACPHD, community partners, and internal BayPLS departments; translate plans and requirements into clear operational actions. Identify actual or potential workflow, implementation, or operational barriers; evaluate alternatives and coordinate solutions within the approved CHIP scope. Coordinate staffing, logistics, site activities, supplies, technology, and other operational needs when required for CHIP meetings, convenings, outreach, assessments, or implementation activities. Conduct site or community visits as required by CHIP activities and provide operational guidance where needed. Use applicable software platforms, including G-Suite/Google Workspace, Microsoft 365, internal databases, and other program or data systems required for CHIP implementation. Implement and adhere to BayPLS and ACPHD policies, procedures, program requirements, safety requirements, privacy expectations, and applicable laws and regulations. Organize and prioritize work to meet critical deadlines and maintain continuity across the CHIP implementation lifecycle. Perform other CHIP-related duties within the approved partnership scope as assigned.
QUALIFICATIONS
Excellent verbal and written communication skills. Demonstrated experience working effectively in a team environment and with external partners or clients. Strong organizational, project coordination, documentation, and follow-through skills. Ability to with software platforms used for communication, project tracking, data management, and reporting, including Google Workspace and Microsoft 365. Ability to travel to meetings, community sites, partner locations, and CHIP activities as required. High School Diploma, General Education Diploma (GED), or equivalent education/experience required. Associate degree preferred, or equivalent training and experience. At least 1 year of administrative leadership, program coordination, or comparable experience preferred. Customer service and stakeholder-facing experience strongly preferred. Experience supporting community health, public health, healthcare, community engagement, government contracts, or grant/contract deliverables preferred.
PHYSICAL DEMANDS/WORKING ENVIRONMENT
Hours and days may vary depending on CHIP implementation and partnership needs. Standard hours are Monday through Friday, 8:00 a.m. to 5:00 p.m., with occasional evening or weekend availability for community meetings, convenings, outreach, or other CHIP activities. Travel within Alameda County and the surrounding Bay Area may be required. The role includes office/computer work as well as attendance at community and partner sites.
