Job Description

Hospital Liaison Job Summary:
To support business development programs designed to drive brand awareness, educate key constituents, recruit resources, increase referrals/admissions/conversion rate and with a primary focus within the hospital setting. Sustain and promote business partnerships to generate early and appropriate referrals within the hospital setting with an extension to Community Partners as encountered in this role. Establish working relationships and open lines of communication between appropriate personnel within NorthStar Care Community (NSCC), patients and families, physicians, discharge planners, nursing and facility staff. Coordinates and plans admissions and provides skilled nursing assessment and care to the patient as prescribed by the attending physician or designee, in compliance with NSCC policies and procedures.
