Case Manager

Approximately 18 hours per week (school year) Hours may vary based on client census and program needs

Schedule:

After-school and evening hours Position Overview We are seeking a compassionate and reliable Case Manager to provide mentoring, youth advocacy, case management, documentation, and transportation services for youth participants. This role supports students and families while ensuring progress toward individualized treatment goals and program requirements. Paid time begins when you pick up a client or arrive at the center and ends when the client is dropped off or all students have departed. Schedule School Year Monday

Thursday:

3 hours after school Meetings on Thursdays weekly in the mornings Summer (Subject to Change)

•Activities, Group Key Responsibilities Provide youth case management, mentoring, and advocacy Attend Early Warning meetings on Thursday mornings Build 1:1 supportive relationships with students Serve as a positive role model and trusted adult Develop, implement, and monitor individualized treatment plans Track academic progress, school attendance, and engagement Facilitate tutoring, career guidance, and life-skills support Provide transportation to and from program activities Arrange drug screens when required Document evidence-based education and group activities Complete daily service logs, monthly progress notes, and required reports Manage weekly schedules and ensure required service hours are met Communicate concerns to Program Director and Social Worker Maintain confidentiality and follow program policies Qualifications High School Diploma or GED required This position requires a background check Associate or Bachelor's degree preferred Minimum 3 years of experience working with youth Valid driver's license Reliable, safe vehicle for transporting clients Current auto insurance required

$20.00 per hour
