Discharge Planner Role:
Hours are 8:00-5:00pm, Monday-FridayMay require an adjustment of schedule to complete assignments and workload.

This position is based within Jefferson RegionalIn this unique setting, daily interactions with a diverse clientele, including hospital staff, physicians, and visitors, are expected.
Job Summary:
The Discharge Planner (DCP) responsibilities are to ensure effective prioritization of tasks, maximize patient safety and satisfaction, and deliver quality care while minimizing negative financial outcomes. Assists with patients who have complex psychosocial needs, require assistance with eligibility determination for programs and funding sources and qualify for community assistance from a variety of special funds and agencies. Assists with crisis intervention to patients and families with social needs. Assists with the discharge plan of care for high-risk patient populations and makes appropriate referrals.
Discharge Planner Qualifications:
High School diploma or equivalentBachelors Degree OR Master's Degree Social Work (MSW)AR State Licensure as a Social Worker.

A minimum of three (3) years experience in hospital or health related setting preferred.

Strong interpersonal and communication skills Ability to assess complex needs and develop practical solutions.

Detail-oriented, with strong organizational and time management skills.

Ability to work effectively in a multidisciplinary team environment.

Ability to communicate with patients, families, staff & physicians.

Ability to work with people of all social, economic, and cultural backgrounds.

Current working knowledge of available community services preferred.

Prior experience in discharge planning, case management, or social work a plus.

Familiarity with community resources, healthcare services, and insurance requirements preferred.
