Napa Post Acute is seeking an experienced, compassionate, and highly organized Social Services Director to lead our Social Services department. This is an excellent opportunity to join a dedicated interdisciplinary team committed to delivering exceptional care and support to our residents and their families. As our Social Services Director, you will play a critical leadership role in enhancing residents' quality of life, supporting successful care transitions, and ensuring compliance with all state and federal regulations. This role ensures that residents' psychosocial, emotional, and discharge planning needs are met while promoting resident rights, dignity, and quality of life. Responsibilities Direct and oversee all Social Services functions within the facility Complete psychosocial assessments and care plans Coordinate discharge planning and community resource referrals Assist residents and families with adjustment, coping, and crisis intervention needs Facilitate resident and family meetings Collaborate with nursing, therapy, physicians, and interdisciplinary team members Ensure compliance with federal regulations, California Title 22 requirements, and facility policies Maintain comprehensive social services documentation Support resident rights, advocacy, and quality of life initiatives Develop and maintain relationships with community agencies and healthcare partners Qualifications Required Bachelor's Degree in Social Work (BSW) or Bachelor's Degree in a related human services field such as Sociology, Psychology, Rehabilitation Counseling, or Gerontology Experience in healthcare social services, skilled nursing, long-term care, hospital, hospice, or related healthcare setting Must meet all applicable California state and federal qualifications for social services personnel in skilled nursing facilities Must meet California Title 22 Social Services Director requirements for a 120-bed skilled nursing facility Strong knowledge of discharge planning, care coordination, and resident advocacy Excellent communication and organizational skills Preferred Skilled Nursing Facility (SNF) experience Previous leadership or supervisory experience in social services California social work licensure, when applicable Schedule Full-Time Monday-Friday Occasional flexibility based on resident and facility needs Join Our Team At Napa Post Acute, we believe exceptional resident care begins with exceptional team members.
