Job Description

Overview We are a resident-centered skilled nursing facility dedicated to providing quality care, dignity, and support to the individuals and families we serve. We are seeking a compassionate, knowledgeable, and motivated Licensed Social Worker to join our interdisciplinary team and make a meaningful difference in the lives of our residents. Duties Conduct psychosocial assessments for residents upon admission and as needed. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Coordinate discharge planning and transitions of care. Provide counseling, support, and crisis intervention for residents and families. Assist residents and families in accessing community resources and financial assistance programs. Participate in care conferences and interdisciplinary team meetings. Ensure compliance with federal, state, and facility regulations regarding social services. Advocate for resident rights and promote resident satisfaction and quality of life. Maintain accurate and timely documentation. Requirements Current Idaho Social Work License (LMSW, LSW, or LCSW preferred). Bachelor's Degree in Social Work required; Master's Degree preferred. Previous experience in skilled nursing, long-term care, healthcare, or geriatric settings preferred. Knowledge of Medicare, Medicaid, discharge planning, and long-term care regulations. Strong communication, organization, and interpersonal skills. Ability to work effectively with residents, families, physicians, and interdisciplinary team members. Pay:
$25.00 - $30.00 per hour
