Job Description

Job Summary We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and resources to individuals and families facing various challenges, including mental health issues, trauma, and social barriers. This role requires a strong commitment to helping clients achieve their goals through effective intervention strategies and advocacy. Responsibilities Conduct thorough patient assessments to identify needs and develop individualized care plans. Provide crisis intervention services to clients in urgent situations, ensuring their safety and well-being. Collaborate with healthcare professionals in hospital settings to coordinate care for patients. Offer specialized support for children with emotional or behavioral challenges, including those in special education programs. Deliver PTSD care and support to individuals affected by trauma. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Collect and analyze data related to client progress and program effectiveness. Advocate for clients' rights and access to necessary resources within the community. Requirements A degree in Social Work or a related field is required; licensure preferred. Experience working with children and families is highly desirable. Knowledge of crisis intervention techniques and patient assessment practices. Familiarity with ICD-9 coding for medical records is a plus. Strong communication skills, both verbal and written, for effective interaction with clients and professionals. Ability to maintain confidentiality and adhere to ethical standards in social work practice. A commitment to ongoing professional development and staying current with best practices in the field.
