A bout Diamond Years Diamond Years Adult Medical Day Care Center has been proudly serving seniors and adults with disabilities in Haledon, NJ since 2016. Our 15,000 sq. ft. facility is licensed for up to 160 participants per shift and features a movie theater, beauty salon, arts & crafts room, dining hall, and outdoor seating. We serve a diverse multilingual community in English, Spanish, and Filipino, with 300+ enrolled clients. Our mission is to make Diamond Years one of the best adult day care centers in New Jersey. Position Overview We are seeking a compassionate Licensed Social Worker to serve as a key advocate for our participants and families — providing psychosocial support, care coordination, and connection to community resources. Key Responsibilities Conduct psychosocial assessments and develop individualized care plans Provide counseling and emotional support to participants and families Assist with Medicaid, Social Security, and community resource navigation Coordinate care transitions and medical appointments Document all social service activities in compliance with NJ state regulations Participate in interdisciplinary team meetings and care plan reviews Support participants through end-of-life planning and bereavement Qualifications BSW required; MSW preferred Active NJ LSW or LCSW license required (or obtainable within 90 days) Experience in adult day care, geriatrics, or healthcare preferred Bilingual English/Spanish strongly preferred Strong documentation and communication skills What We Offer Competitive salary based on experience No nights or weekends Supportive, mission-driven team Health insurance, 401(k), and paid time off A beautiful, active facility you will be proud to work in How to
