The ideal candidate will have experience in hospital settings and a strong understanding of case management, behavioral therapy. This role involves working closely with individuals and families to provide support, resources, and interventions that promote mental health and well-being. The Social Worker will be responsible for assessing client needs, developing treatment plans, and ensuring compliance with HIPAA regulations. Responsibilities Conduct comprehensive assessments of clients' social, emotional, and environmental needs. Develop and implement individualized treatment plans based on diagnostic evaluations. Provide case management services to assist clients in accessing necessary resources. Utilize cognitive behavioral therapy techniques to support clients in achieving their goals. Maintain accurate records of client interactions and progress in compliance with HIPAA standards. Collaborate with healthcare professionals to coordinate care for clients within hospital systems. Advocate for clients with autism and other behavioral health challenges to ensure they receive appropriate services. Requirements Bachelor's or Master's degree in Social Work from an accredited institution. Social Worker wanted for an Adult Day Care . MUST be a Licensed Social worker or Certified Social worker to apply. New Grads with LSW or CSW are welcomed to apply.

Duties include:

Assists clients with accessing social services, including completing Medicaid and SNAP applications. Compiles social history and conducts a psycho-social assessment including formal and informal support systems, mental and emotional status, community and financial resources, and care giver data. Provides discharge planning and assists in the transition and follow-up as appropriate. Contributes to a positive work environment and overall team effort. Performs related duties and responsibilities as required or directed by the administrator.
