Shift Details:

Monday-Friday 8:00am-4:30pm

Social Worker Duties & Responsibilities:

Assessing individuals, developing individual care plans and determining necessary resources. Advocating for and assisting individuals and families in accessing and obtaining services. Contacting service providers and attending interdisciplinary conferences to determine appropriate placement and referrals. Monitoring individual rehabilitation goals and progress. Providing crisis intervention services. Minimum Qualifications LMSW License is required Background check requirements:
