Company Overview Professional Care Services of West TN is a dedicated community mental health agency providing outpatient behavioral health and substance abuse services to children and adults. With multiple locations across the region, our team includes psychiatrists, behavioral health nurses, case managers, therapists, and pharmacists committed to supporting the well-being of diverse populations. Job Overview We are seeking a compassionate and proactive Family Link Navigator/Care Coordinator to join our dynamic team. In this vital role, you will serve as a bridge connecting families with essential child and family services, ensuring they receive the support they need during challenging times. Your energetic and compassionate approach will help empower families, coordinate care plans, and facilitate access to community resources, all while maintaining clear communication and fostering trust with clients from culturally diverse backgrounds. Responsibilities Act as a primary point of contact for families seeking child welfare, behavioral health, or social services support Conduct thorough assessments to identify family needs and develop tailored care plans in collaboration with clients and service providers Coordinate with child protective services, healthcare providers, legal entities, and community organizations to ensure comprehensive support for high-risk populations Provide crisis intervention and crisis management during urgent situations to stabilize families and ensure safety Maintain detailed documentation of client interactions, service plans, and progress notes. Experience Proven experience working with children, families, or individuals with disabilities in social work or child welfare settings Knowledge of and experience as a parent or caregiver of a child with behavioral health needs is required. Skills in crisis intervention, crisis management, and conflict resolution within behavioral health or social services environments Proficiency in Microsoft Office applications for documentation and reporting purposes Strong communication skills—both written and verbal—are essential for client engagement and interagency collaboration Join us in making a meaningful difference by empowering families through compassionate care coordination. Pay:
From $42,000.00 per year
