Full-Time Job Summary The State of Tennessee is seeking a Social Worker 2 / Social Counselor to join the Clinical Care Team supporting primary care clinics throughout the region. This role focuses on helping patients overcome barriers to care by addressing social determinants of health (SDOH), coordinating community resources, and collaborating with healthcare providers to improve patient outcomes. The successful candidate will work directly with patients, healthcare providers, hospitals, and community agencies while participating in regional meetings and travelling within the assigned region as needed. Key Responsibilities Conduct social needs screenings and assessments for patients referred by primary care providers. Assist patients with social determinants of health, including: Food insecurity Housing assistance Transportation Clothing and other essential resources Mental health and community services Develop and maintain community resource directories, including eligibility requirements and referral processes. Coordinate case management services and patient follow-up. Support hospital discharge planning and continuity of care. Assist patients in obtaining medical records and durable medical equipment. Help patients access affordable prescription programs and patient assistance resources. Assist with specialty referrals, scheduling, and coordination when needed. Document patient interactions and care coordination activities within the medical record. Provide patient education and connect individuals with appropriate health resources. Attend provider meetings, community health councils, and regional clinical meetings. Identify barriers to healthcare access and collaborate on solutions. Minimum Qualifications Bachelor's/Master's/Doctorate degree in Social Work or Bachelor's degree with relevant experience in social work, counselling, healthcare, mental health, or related fields. Minimum 1 year of professional experience required (qualifying graduate coursework may substitute).
CMSW/LCSW
certification or relevant social counselling experience is preferred/accepted as applicable. Required Skills Minimum 1 year of experience supporting patients with Social Determinants of Health (SDOH) such as food insecurity, housing insecurity, or community resource navigation. Strong case management and care coordination skills. Excellent communication and interpersonal abilities. Ability to collaborate effectively with multidisciplinary healthcare teams. Strong documentation and organisational skills. Proficiency with computers and electronic documentation systems.
