Overview Join our dedicated healthcare team as a Social Worker/Case Manager, where you will play a vital role in providing compassionate support and comprehensive case management services to diverse patient populations. This dynamic position offers the opportunity to collaborate with interdisciplinary teams, coordinate community resources, and deliver personalized care that enhances patient outcomes. Duties Conduct thorough patient assessments to identify social, emotional, and environmental needs, ensuring adherence to clinical ethical guidelines and HIPAA regulations. Develop individualized care plans that incorporate behavioral therapy, motivational interviewing, and crisis intervention techniques tailored to each client's unique circumstances. Coordinate community resources, including hospice care, assisted living, mental health services, and developmental disability programs, to support patient needs across settings. Provide clinical counseling and group therapy sessions for patients and families dealing with end-of-life issues, trauma, addiction, or behavioral health challenges. Facilitate discharge planning by collaborating with medical teams to ensure smooth transitions from inpatient or outpatient care settings. Maintain accurate medical documentation and medical records using electronic medical record (EMR) systems while complying with HIPAA standards. Engage in ongoing professional development through research, clinical interprofessional collaboration, and applying evidence-based practices such as cognitive behavioral therapy (CBT) and applied behavior analysis (ABA). Qualifications Bachelor's or Master's degree in social work, psychology, counseling or related field; licensure or certification preferred. Proven experience in hospital settings, hospice care, mental health counseling, or community-based social work practice. Familiarity with medical terminology, CPT coding, ICD-9/10 coding systems, and Medicare regulations. Strong skills in client communication, diagnostic evaluation, crisis management, and case management. Experience working with individuals from diverse cultural backgrounds and with disabilities such as autism or developmental delays is highly valued. Knowledge of EMR systems and medical documentation standards is required; inpatient or nursing home experience is a plus. Ability to handle sensitive situations with empathy while maintaining professional boundaries under clinical ethical guidelines. Certifications in CPR/BLS are required; additional certifications in hospice & palliative medicine or addiction counseling are advantageous.
