Days Job Responsibilities:

Conduct social needs screenings and assessments for patients referred by primary care providers. Assist patients in accessing community resources, including food, housing, transportation, clothing, and mental health services. Provide case management, discharge planning, patient education, and follow-up care. Coordinate with physicians, nurses, social workers, and community agencies to ensure continuity of care. Help patients obtain medical records, medical equipment, and specialty referrals. Assist patients with medication assistance programs and low-cost prescription resources. Maintain accurate documentation in the electronic medical record. Participate in community outreach and identify barriers to healthcare access.

Facility Requirements:

Bachelor's or master's degree in social work or a related field. Experience in case management, care coordination, social services, or community health. Knowledge of Social Determinants of Health (SDOH) and community resource navigation. #
