DESCRIPTION
The Social Work Care Manager I works closely with the interdisciplinary team to facilitate discharge planning and address the complex care needs of patients who are at risk of adverse outcomes due to limited resources, complex family dynamics, diagnosed or suspected mental health issues, diagnosed or suspected substance use disorders, difficulty navigating systems of care, involvement with or suspected need to involve protective services, suspected domestic abuse, difficulty navigating systems of care, ethical concerns, or other psychosocial complexities as needed and appropriate. The SW Care Manager I performs mental health assessments, evidence-based interventions (SBIRT) for those at risk of substance use disorder or those diagnosed with substance use disorder, cognitive function screenings, crisis interventions, safety planning, and/or other appropriate interventions to patients with psychosocial needs. The SW Care Manager I works closely with Nurse Care Managers, physicians, nursing staff, and other staff as appropriate to determine when social work involvement is indicated to address patient care needs.
EDUCATION
Masters of Social Work required.
EXPERIENCE
Previous experience in medical social work preferred. Previous experience in working with substance use disorder, behavioral health, end of life care, adults with housing insecurity, older adults, vulnerable adults.
LICENSURE/CREDENTIALS
Basic Life Support (BLS) certification required within 30 days of hire. SBIRT certification required within six months of hire.
