Social Worker/Discharge Planner

Job Description

POSITION PURPOSE
Assesses patient's medical condition, psychosocial concerns, home environment and additional information to provide appropriate discharge placement. Communicates with family and/or significant other, staff and outside agencies to discuss discharge-planning alternatives. Assists patients/ families in coping skills and provides emotional support. Prepares consultation reports and documents activity in patient medical record. Performs other related duties as needed.
MINIMUM EDUCATION REQUIRED
Bachelors Degree in: Social Work or related field.
MINIMUM EXPERIENCE REQUIRED
Entry Level
LICENSES / CERTIFICATIONS REQUIRED
Licensure to be eligible to practice as a Social Worker in the State of Wisconsin or ability to obtain license within 12 months of hire.
KNOWLEDGE, SKILLS & ABILITIES REQUIRED
Social work experience in the health care arena with previous clinical experience preferred. Must be able to read, write, speak, hear and comprehend the English language.
PRINCIPLE ACCOUNTABILITIES AND ESSENTIAL DUTIES
Performs patient medical and psychosocial assessment for development of discharge plan. Communicates with multidisciplinary team. Documents in the patients' medical record. Assists patients/families in coping skills and provides emotional support. Knowledge of and ability to discuss financial alternatives with patients and families. Participates and leads interdisciplinary meetings. Serves as a resource person for the multidisciplinary team. Is a liaison between United Hospital System, Inc. and external referral sources. Attends and presents in-services and educational programs in order to maintain and enhance professional expertise. Receives and records verbal orders from licensed independent practitioners for orders that are specific to the services provided by their department.
