Job Description

Requirements:

Licensed Clinical Social Worker must be licensed by the California Board of Behavioral Sciences. Must possess current CPR and First Aid Certification. TB clearance and health examination from a physician (at time of hire or within 7 days of hire).

Job Description:

Participate in the initial and continuing assessments for the development of an individual plan of care by the multidisciplinary team.

Consultation services are indicated when:

Provide treatment in accordance with the plan of care and document services provided. Maintenance of signed and dated progress records in the participant's health record when indicated. Liaison with other members of the multidisciplinary team, family members, and referral agencies that may yield information for psychiatric or psychological treatment. Group counseling and techniques, as indicated by participants' needs. Provide a minimum of three hours per month for in-person staff consultation/training regarding psychosocial needs of participants.
