Job Summary We are seeking a Spanish Speaking Medical Social Worker to join our healthcare team. The ideal candidate will provide essential support to our program participants and their care-givers, ensuring they receive the necessary resources and assistance to cope with medical conditions and navigate the healthcare system effectively. We will train new MSW grads! Duties

• Conduct assessments to develop and implement social work/ care coordination care plans
• Assist program participants in accessing community resources
• Maintain accurate documentation of social services interactions with the program participant
• Provide case management services to coordinate patient care and support services
• Facilitate individual or group therapy sessions
• Collect and analyze data related to social services outcomes and treatment efficacy Qualifications
• Master of Social Work degree
• Fluent in Spanish
• Experience in social work, care planning, and patient assessment (We will train new MSW grads)
• Proficiency in behavioral therapy, patient care, and medical documentation (We will train new MSW grads)
• Knowledge of HIPAA regulations and medical terminology (We will train new MSW grads)
• Ability to communicate effectively with diverse populations
