Job Overview We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) and Licensed Master Social Worker (LMSW) to join our dynamic healthcare team. Responsibilities Conduct thorough mental health and behavioral assessments utilizing clinical documentation standards and medical terminology to inform diagnosis and treatment planning. Deliver evidence-based interventions such as cognitive behavioral therapy, psychodynamic therapy, applied behavior analysis, and other modalities tailored to individual needs. Provide individual and group counseling sessions focused on mental health counseling, addiction counseling, PTSD care, autism spectrum disorders, developmental disabilities, and geriatrics. Provide Targeted Case Management (TCM) services to eligible individuals, including comprehensive needs assessments, development and implementation of individualized service plans, care coordination, referrals to community and healthcare resources, monitoring of services, and ongoing follow-up to address identified needs and barriers to care. Provide Case Management for Children and Pregnant Women (CPW) services to eligible Medicaid members, including children and high-risk pregnant individuals who have health-related conditions and require assistance accessing medically necessary health and health-related services. Identify and address health, social, educational, behavioral, and resource barriers that may interfere with a client's ability to access or maintain necessary healthcare and support services. Coordinate with clients, families, physicians, behavioral health providers, therapists, schools, Medicaid managed care organizations, community agencies, and other members of the care team to facilitate access to appropriate services and continuity of care. Assist clients and families with navigating healthcare systems, community resources, referrals, service access, and authorization-related barriers when appropriate. Conduct ongoing monitoring and follow-up to evaluate whether case management services and referrals are meeting identified client needs and modify service plans as necessary. Maintain timely, accurate, and compliant documentation of TCM and CPW Case Management activities , including assessments, service plans, referrals, care coordination activities, client contacts, follow-up efforts, and outcomes in accordance with applicable Medicaid, payer, organizational, and regulatory requirements. Collaborate with multidisciplinary teams to develop comprehensive case management plans that include patient care coordination within hospital or community settings. Maintain accurate clinical documentation in electronic medical records (EMR/EHR) systems while adhering to HIPAA regulations and clinical confidentiality policies. Engage in crisis management and crisis intervention efforts to support patients experiencing acute mental health or behavioral emergencies. Collect data for clinical research projects related to mental health interventions or behavioral health outcomes; contribute to ongoing quality improvement initiatives. Skills Proven experience in psychotherapy, mental health assessments, and clinical social work practices across diverse populations. Strong knowledge of motivational interviewing techniques and evidence-based practices for behavioral health interventions. Expertise in child & family counseling, special education considerations, autism spectrum disorder interventions, and working with individuals with disabilities or developmental delays. Knowledge of Targeted Case Management, care coordination, service planning, community resource navigation, and Medicaid-funded case management services is preferred. Ability to assess social determinants of health and identify barriers affecting access to medical, behavioral health, educational, social, and community-based services. Familiarity with EMR systems (Electronic Medical Records/Electronic Health Records), CPT coding, ICD-9/10 coding standards for accurate medical documentation and billing purposes. Experience working within hospital settings or home & community care environments; familiarity with crisis management and psychiatric patient care protocols. Ability to perform diagnostic evaluations for mental health conditions such as PTSD, substance use disorders, Alzheimer's care, or other neurocognitive disorders. Strong communication skills for client engagement, patient assessment, care coordination, and collaboration with healthcare teams, families, community organizations, and individuals from diverse cultural backgrounds.
