Job Summary We are seeking a compassionate and dedicated Licensed Medical Social Worker specializing in hospice care to join our team. Responsibilities Conduct thorough patient assessments to evaluate emotional, social, and psychological needs within the context of hospice and palliative medicine. Develop individualized care plans that incorporate social work practice standards, community resource coordination, and end-of-life care principles. Provide clinical counseling, including grief support, behavioral therapy, mental health counseling, and crisis intervention tailored to patients and families. Facilitate communication between patients, families, healthcare teams, and community resources to ensure seamless discharge planning and ongoing support. Maintain accurate medical documentation in electronic medical records (EMR) systems while adhering to HIPAA regulations and clinical ethical guidelines. Collaborate with interdisciplinary teams through clinical interprofessional collaboration to optimize patient-centered care. Educate patients and families about available community resources, hospice services, Medicare policies, and medical terminology relevant to their care journey. Skills Proven experience working with geriatric patients, individuals with disabilities, dementia care, Alzheimer's care, PTSD care, or developmental disabilities. Strong background in hospice care, end-of-life support, crisis management, and post-acute care experience. Proficiency in medical documentation systems (EMR), ICD coding, and medical records management. Knowledge of hospice & palliative medicine standards alongside clinical research methodologies related to behavioral health. Ability to perform diagnostic evaluations, case management duties, intake assessments, and group or individual counseling sessions. Excellent client communication skills with a focus on motivational interviewing and cultural competence when working with diverse populations. Familiarity with hospital experience, inpatient settings, outpatient clinics, assisted living facilities, home & community care environments. Certifications in basic life support (BLS), CPR, and familiarity with medical terminology essential for safe patient care delivery.
